Your insurance carrier may or may not reimburse you for medical services provided by us, depending on your individual policy. It is your responsibility to check on your medical benefits before we provide you with the service. You need to know if you are required to have a second opinion prior to surgery or precertification for hospitalization. Our business office staff is prepared to discuss our fees for your specific medical needs.
Once medical services are rendered, payment in full is expected. You, not your insurance carrier, are responsible for payment of our fees. Should your insurance carrier's payment be different from our fees, that is a matter between you and your insurance carrier. At the time of your clinic visit, you will be provided with a statement which contains all the information necessary for you to file your claim with your insurance company. As a courtesy we will directly file your Medicare and/or insurance claim at your request. Our office is prepared to assist you in completing your insurance forms so that you may receive your maximum insurance benefits.
No information in your medical record will be released by our office to insurance companies, attorneys or anyone without your written consent.
